Family Assessment Form
Assessment is essential to service delivery. Measures are needed that enable practitioners to capture family circumstances and the complex transactions among family members. The Family
Assessment Form (FAF) measures the following six areas critical to a well-functioning family:
- Living Conditions
- Financial Conditions
- Support to Caregivers
- Caregiver/Child Interactions
- Developmental Stimulation
- Caregiver Interactions
The FAF also helps evaluate and measure the impact programs and services have on families served.
The FAF was developed by Children's Bureau and is now published and distributed by the Child Welfare League of America. The FAF is designed as a standardized form, but can be adapted in its
implementation to a variety of clinical, procedural and/or program needs.
